Privacy Act Rights
Alpha follows the requirements of the Privacy Act, which provides a number of protections for your personal information. Under the Health Insurance Portability and Accountability Act if 1996 (HIPAA) and the HITECH Act, we have an obligation to implement reasonable safeguards to protect the privacy of our claimants’ PHI/PII while providing appropriate access to those same records.
It is the policy of Alpha to:
• Maintain the confidentiality of individual’s PHI/PII stored or transmitted in any form or medium (electronic, paper or oral) through new hire training and ongoing education of staff on the proper use of claimant information in the delivery of Services as outlined in the Service Agreements between Alpha and its client(s).
• Collect the minimum necessary level of PHI/PII information from our Health Plan clients, their enrolled members and our individual claimants as required to complete our responsibilities.
• Verify the identity of the individuals with whom we speak to ensure the accuracy of the information we received but at no time do we disclose, except to the appropriate agencies, including the Veterans Administration (VA) and only with the required signed authorizations, any information collected. PHI will be disclosed only to persons who are authorized to receive it. Appropriate statements or documents confirming the identity of the persons requesting PHI are required as a condition of disclosure.
Upon hire, all new employees must attend the mandatory HIPAA Privacy and Security training. In addition, re-training occurs annually. In both instances, a test is given to demonstrate understanding of the information presented.
We will not disclose your personal information to third parties outside this organization without your consent, except to facilitate the transaction, to act on your behalf at your request, or as authorized by law.
We will use your information to process requests for services or information. Providing the information is generally voluntary, but if it is not provided, we may not be able to process your request. When information is required to process your request, we will advise you of this fact.
Information Collected from Online Forms
A few of the pages on our website allow you to use online forms. We will use the information you provide only to respond to you or process a request. If your email communication or online form contains sensitive information know that such communications are appropriately secured for transmission over the Internet.
Alpha’s security policies are based on an assessment of risks to the confidentiality, availability, and integrity of information we maintain. Security policies, and specific security procedures developed to implement those policies, are based on that risk assessment, and an analysis of the probability and criticality of identified risks, the costs of security measures, technical resources available, and our business needs.
It is the policy of Alpha to apply sanctions consistently for privacy-related violations. Failure to comply with this policy will call for disciplinary action.
We do not sell, rent, or otherwise provide your personal information to outside marketers. Information collected via this website may be shared with employees or Advocates as necessary to respond to a request, provide a service, or as otherwise authorized by law.
